NeuroRestorative’s home and community programs bridge the gap between inpatient rehabilitation and regained independence by offering services and supports both in an individual’s home and in the community.  We provide one-on-one rehabilitation focusing on activities of daily living, community reintegration, social and emotional growth, and vocational goals.  In addition, we offer school re-entry and supported employment services.

Our team of rehabilitation professionals work together with the individual and their family to develop an Individual Rehabilitation Plan (IRP) that includes goals, interventions, outcomes and supports. Each individual is assigned a highly trained clinical coordinator who provides therapeutic case management and service coordination.

Our home and community programs are delivered in the individual’s home, community and/or work setting to help reestablish a long-term support network in a familiar setting.
