To Make a Referral
- We accept referrals for medically-stable individuals six years of age or older.
- We use the Rancho Los Amigos scale to assist in determining appropriate referrals. (Please refer to scale below.)
Referral Process:
- Please call our referral coordinator at (800) 743-6802 or click here to email a referral coordinator.
- Immediately upon receipt of your referral we will contact you to gather important information about the patient; schedule a clinical evaluation; and arrange to meet with family and/or others involved with the patient. In most cases, the clinical evaluation can be scheduled within 48 hours after the referral is made.
- Upon clinical acceptance, and following clinical evaluation, a proposed plan of care is provided to all authorized parties.
- Our referral coordinator will work at securing funding qualification, arranging a family tour, and communicating with all parties.
- Upon final funding approval our referral coordinator will assist with arrangements to transfer and admit the patient.
For more information, please call our referral department at (800) 743-6802.
The Rancho Los Amigos scale is an important guideline in determining appropriate referrals. Individuals who are medically stable and are a Rancho IV or higher would be considered for admission.
