We provide home and community services in the following counties: Allegheny, Armstrong, Beaver, Berks, Bucks, Butler, Chester, Dauphin, Delaware, Erie, Fayette, Greene, Lancaster, Lawrence, Lebanon, Lehigh, Montgomery, Northampton, Northumberland, Philadelphia, Schuylkill, Washington, Westmoreland, and York.

Designed to occur in the home, school, workplace, or other locations within the community, our Cognitive Rehabilitation Therapy (CRT) model allows for skill development in a variety of environments to allow for generalization and broader application of skills and strategies. The goal of services is independent living in the individual’s home and community of choice. CRT services can be provided alone or in combination with other services. For individuals living in more rural areas, tele-rehabilitation services may be provided.

CRT is a one-to-one specialized therapy that addresses the cognitive deficits that result from an injury to the brain (stroke, traumatic brain injury, meningitis, etc.) or condition that results in problems with learning (multiple sclerosis, autism, developmental disability). Our Integrated Model focuses on providing education and skills training, developing compensatory strategies, and assisting the individual with applying the skills and strategies in everyday activities.

With a specialization in CRT, our program integrates the latest research with established clinical practices to produce strong outcomes for increased independence and community participation, as well as improved quality of life.
